Our Programs

Transition House offers the following services to address the traumatic and destabilizing impact of family violence: emergency shelter, transitional housing, and permanent supported housing all with intensive holistic support services; a range of intervention and outreach services including safety planning, counseling, support groups, court accompaniment, referrals to legal services, medical and mental health care; and youth violence prevention resources in schools and in out of school time. To maximize our impact, we work with a variety of community partners to provide these services in diverse and culturally sensitive settings that support and empower families to live safely, access community resources, education, training and sustainable employment that will set the stage for a future free from domestic violence.

Our work strengthens families through two key strategies: (1) Housing Programs for families displaced from their homes by domestic violence that help survivors rebuild their lives and access permanent and sustainable homes; and (2) our Community Support Partnership which delivers education resources, support, and counseling services in community settings with programs designed to strengthen families by preventing homelessness and supporting parent and child stability.
